Our registered managers will visit you at home (or in hospital if necessary) to conduct a pre-service assessment. This is a friendly, no-pressure conversation about your needs.

3. Personalized Support Plan

We’ll document your needs and create a support plan showing how our skills can improve your health and wellbeing. This will be done with your full knowledge and cooperation.

4. Transparency & Access

  • All assessments are made with your full knowledge
  • Records will be shown to you
  • You have access to your records at any time
  • You can request reviews whenever needed

5. Service Commencement

We manage the start of services according to detailed policies and procedures. A copy is available on request.
